Fall sports physicals will be held Wednesday August 5, 2009 beginning at 8:00 am. Physicals will be held in the high school nurse's office. Physical forms may be picked up in the main office or you can download the form under athletic forms on the side bar of this web page. Please complete the first four pages and turn them in to your coach or Mr. Dupont. All forms must be completed prior to having a physical. If the forms are incomplete you may not receive a physical.
